You have 200 survey responses back, the data looks clean, and you still cannot answer the question you started with. In most cases, the format was wrong from the start, not the questions themselves. Choosing the right type of questionnaire in research is the decision that determines whether your data is usable before you write a single question.

What counts as a questionnaire?

A questionnaire is a set of written questions used to collect data from respondents. The term is often used interchangeably with “survey,” though technically a questionnaire is the instrument itself (the list of questions), while a survey is the broader process of data collection.
Questionnaires can be delivered on paper, online, or verbally in interview settings. Online questionnaires have become the dominant format for organizational research: they cost less, reach larger samples, and allow logic branching that paper can't support [1]. The tradeoff is lower response rates, something worth factoring into your sample size planning.

Even well-intentioned questionnaire designs go sideways. These are the four mistakes that show up most often in organizational research, and how to sidestep them.
Using the wrong type for your goal
The most frequent error is defaulting to a structured questionnaire simply because it feels more “professional” or is easier to analyze. If you genuinely don't know what's driving a drop in engagement scores or why a training program isn't landing, a closed-ended rating scale won't give you an answer. If the goal is diagnostic or exploratory, add open-ended questions even if they take longer to analyze.
Asking leading questions
A leading question nudges respondents toward a particular answer. “How satisfied were you with the excellent facilitation?” is a classic example, since the word “excellent” signals the expected response before the question is answered. Leading questions inflate positive responses and produce data that isn't trustworthy. Review every question for loaded language and test with a small pilot group before full rollout. If reviewers consistently answer the question the same way regardless of their actual experience, it's probably leading.
Making questionnaires too long
Length is one of the most reliable predictors of completion rate. Researchers sometimes add questions “just in case” they're useful later, which gradually turns a 6-question pulse survey into a 20-question marathon. Every question should earn its place by connecting directly to a decision you'll make with the data. If you can't name what you'll do differently based on a question's responses, cut it. For most workplace contexts, aim for under 10 questions and a completion time under 4 minutes.
Neglecting response scale consistency
When a questionnaire mixes different scale formats, such as a 5-point scale for one section, a 10-point scale for another, and a 3-option rating for a third, respondents have to mentally recalibrate at each question. This increases cognitive load and introduces inconsistency in how scales are interpreted. Pick one scale format for your structured questions and use it throughout. If your organization benchmarks against external data (like industry engagement norms), use a scale that's compatible with the benchmark rather than designing from scratch. Label every point on the scale clearly. “Agree” and “Disagree” at opposite ends leaves the middle points open to interpretation; labeling each option removes that ambiguity and produces cleaner data.
